Hospice Nurse Practitioner

As a Hospice Nurse Practitioner, you have a unique role and will serve as a primary provider for a patient’s ever-changing medical information, helping ensure each patient can get the care they need, when they need it. Utilize your high level of medical knowledge, advanced hospice nursing skills, and compassion to the work with the focus on enhancing quality of life for patients knowing they are approaching the end of life. You will collaborate with other members of the hospice care team, including physicians, nursing staff, social workers, and spiritual advisers.
Your Responsibilities:
- Serve as the Primary Care provider within the scope of practice of the Nurse Practitioner.
- Diagnose, treat, and manage patients and their families to ensure they receive the best possible care.
- Conduct face to face visits to assure the continued medical eligible of the hospice patient.
- Ability to accurately practice pharmacological interventions based on the needs of the individual hospice patient.
- Communicates with interdisciplinary team (RNCM, Social and Chaplain) changes in client’s medication, medical status and psychosocial that optimize patient comfort and is consistent with patient and family needs and goals.
- Participates as a member of the Minnesota Hospice care team and attends all IDT meetings.
- Prepare and record accurate medical symptoms and patient health history in a timely manner.
- Ability to be available to support RN On-Call staff regarding medications.
Required Knowledge, Skills, and Experience:
- Strong listening skills, patience, and empathy to relate to patients and families.
- Professional and teamwork oriented, with a focus on quality care.
- Excellent attention to detail
- Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
- Bachelor of Science in Nursing (BSN) Degree or equivalent
- Graduate Degree in Nursing
- Advanced certification from an accredited national certification organization
- Advanced Practice Registered Nurses (APRNs) preferred.

Salary: $105,000.00 – $120,000.00 per year
