We are an independent, local organization of dedicated and experienced professionals specializing in end-of-life care. We service hospice patients in many areas across the Twin Cities, this includes the following counties and cities and surrounding areas: Dakota County, Hennepin County, Rice County and Scott County. We service patients at their locations in Apple Valley, Belle Plaine, Bloomington, Burnsville, Eagan, Edina, Elko New Market, Faribault, Farmington, Hastings, Inver Grove Heights, Jordan, Lakeville (our headquarters), Lonsdale, Mendota Heights, New Prague, Northfield, Prior Lake, Richfield, Rosemount, Savage, Shakopee, South St. Paul and West St. Paul.
