About Minnesota Hospice
Minnesota Hospice was founded by local physicians committed to improving the model of hospice care in their community. We are an independent, local organization of dedicated and experienced professionals specializing in end-of-life care. We understand the decision to begin hospice defines how you live the final chapter of life. Our priority is to support your decisions on how that precious time is lived. To us, that means getting to know you and your loved ones and tailoring our care to your needs. For you, it means experiencing the highest quality of life on your terms.
We service hospice patients in many areas across the Twin Cities, this includes the following counties and cities and surrounding areas: Dakota County, Hennepin County, Rice County and Scott County. We service patients at their locations in Apple Valley, Belle Plaine, Bloomington, Burnsville, Eagan, Edina, Elko New Market, Faribault, Farmington, Hastings, Inver Grove Heights, Jordan, Lakeville (our headquarters), Lonsdale, Mendota Heights, New Prague, Northfield, Prior Lake, Richfield, Rosemount, Savage, Shakopee, South St. Paul and West St. Paul.

Gold Standard of Care
1) Culture
Our team and community culture are our most valuable assets. We share a primary mission centered upon patient-focused care and well-being. That mission extends into each home and residential community with whom we partner and to the patients we serve.
2) Quality of Life at End of Life
Comfort, dignity, and choice are afforded to all those we serve.
3) The Golden Rule
The principle of treating others as one would wish to be treated oneself is the standard for how we serve.
4) Education
We continually update our learning and redefine industry standards to raise the level of understanding of hospice best practices for those with whom we partner and serve.
5) Regulatory Compliance
We comply with applicable laws, regulations, and professional standards of practice. We implement systems and processes to ensure legal, ethical, and moral actions as defined by business best practices and principles of healthcare.
